New patient forms, surgical instructions and other documents are available for download below. The documents are in Adobe PDF format and can be viewed with free Adobe Reader software. For New Patients

As a new patient, you may save time by completing the following forms prior to arriving for your appointment with Dr. Davis. Please note that there is a non-refundable consultation fee of $250 due at the time of the appointment. This fee is not applicable toward any future surgery.

Surgical Instructions
